Both New and established clients may schedule appointments

 

If you are a NEW client, please watch your email for a link to our client portal for access to our office forms which are required for new clients. Be sure to check your spam folder as they sometimes end up there.

 

FORMS MUST BE COMPLETED 24 HOURS PRIOR TO APPOINTMENT OR YOUR APPOINTMENT WILL BE CANCELLED! 

 

If you are unable to fill out the forms online prior to your appointment, please call our office manager, Misti, at 205-572-9886 to to complete forms over the phone at least 24 hours prior to appointment. Please be sure you are familiar with your insurance copay and deductible. Your copay will be payable at your first visit. If you have not met your deductible for the year, you may be responsible for our contracted fee. Please bring your insurance card and driver's license to your first appointment or upload them to your client portal.
